Please see information below re. insurance coverage. I accept Blue Cross Blue Shield (BCBS) primary insurance and cash pay at this time. I do not take Medicare or Medicaid products and I am unable to bill BCBS as a secondary insurance. I do accept FSAs and HSAs, and I can provide superbills for out-of-network reimbursement.

Yes, I work with people ages 6-64.

I offer in person appointments at my office in Pittsfield, Massachusetts, and telehealth appointments across Massachusetts. All clients must be located in the state of Massachusetts at the time of our appointment, per state licensing regulations.

I do not prescribe any controlled sleep medications, such as Ambien or Lunesta. I also do not prescribe any benzodiazepines, such as Xanax, Ativan, or Klonopin. If you are interested in very carefully tapering off these types of medications though, I’d be happy to make recommendations.

Missed appointments and appointments cancelled with less than 24 hours notice will be billed the full visit fee. Patients with three late cancellations or missed appointments will be discharged from the practice.

About Your Appointments and Billing
At The Rest Practice, my standard of care for mental health and sleep care includes both medication evaluation/management and brief supportive psychotherapy. This integrated approach allows me to provide thorough, whole-person care during your appointments, which typically run 30–60 minutes depending on your needs.
Because both services are provided, I bill insurance for two services at most appointments:
∙ Psychiatric medication management (CPT code: 99212–99215, depending on complexity)
∙ Psychotherapy add-on (CPT code: 90833 or 90836, depending on the length of psychotherapy provided during your visit)
For initial evaluations (first visits), I bill one of the following depending on the nature and complexity of your appointment:
∙ 90792 – Psychiatric diagnostic evaluation with medical services
∙ 99204 – New patient office visit, moderate complexity
∙ 99205 – New patient office visit, high complexity
To avoid any surprises, I encourage you to contact your insurance company before your appointment and ask the following:
“I have an appointment with a psychiatric nurse practitioner named Kimberly Loring. Her NPI-1 number is 1083198642, and her NPI-2 number is 1346035094. She will bill CPT codes 90792, 99204, or 99205 for an initial evaluation, and CPT codes 99213 - 99215 plus 90833 or 90836 for follow-up appointments. Are these covered under my plan, and what will my copay or coinsurance be for each?”
Your out-of-pocket responsibility depends on your specific insurance plan.

Initial Evaluation
I will review your completed intake forms prior to your first appointment with me. We’ll start by discussing what’s going on right now and take a closer look at your medical and mental health history. Together, we’ll explore options like medication, labs, therapy, or lifestyle changes and then map out a plan for ongoing treatment and monitoring.

Extended Follow Up Visits
These visits offer you more time if you're in the early stages of treatment, facing increased life stress, dealing with severe symptoms, or managing medication side effects. They're also ideal for those who are dedicated to regular talk therapy or making lifestyle changes to enhance their mental health. Rest Practices (see details below) may be included in these visits, if they are part of your treatment plan.
60 minutes | $200 (self-pay rate)

Standard Follow Up Visits
I believe at least 30 minutes are necessary for follow-up visits to provide the detailed and personalized care you deserve. We’ll talk about any changes in your life, your symptoms, and your medications, and from there, adjust your plan to help you feel better.
